We are all seeing smiles everywhere in OP, from pirate´s simble to tatooes, and i think it would be fun to try finding out what ODA (all hail his name) means by all this (I know its hard but still, fun)... I can think of 3 possibilities:

1) They mean nothing and Oda is just playing with our minds.
Very unlikelly seeing how many smiles have popped up trought the series, lets see:
Binnamy, Doflamingo, Ace´s hat, at the auction house (doflamingo again i know), Kidd, Law, hell even Whitebeard has one http://www.onemanga.com/One_Piece/234/12-13/ (top left, on the serum thing...), i may have forgot some.
As i said unlikelly, but still possible

2) They relate to Roger´s execution.
As we know, the pirate king died with a smile on his face, so all the pirates, who admires his way of thinking and acting, wear some kind of smile in his honor.
Doflamingo wants to make everyone forget the old way of the pirates and create the "new age", therefor he is "anti-smile" and thats the reason behinde his sign.
This is the most accepted theory, but i dont like it, because its to predictable for ODA. Also, if that was the case, Luffi would defenetly wear one!!

3) There is some kind of secret society of pirates and the smile is the sign
Crazy uhn!?!? Well i think there is a community who secretly help pirates, with information, supplies, or any other way to finish the GL. Maybe some powerfull ex-pirates are behind it, or maybe current ones, and there are meetings, to recrute new members, and make plans.
Something like what warriors in the middle age used to do. Even thought they were in enemy armies outside, they respected each others strenght while inside, and, lived theire lifes following a set of rules they established among themselfes.
I´m thinking that Doflamingo was expelled from it, and now is against anything the society has decided.
I know there are a little holes in my theory, but u get the point, and im sure ODA could make something like that, but perfect as allways

Well what do u think of this theories?? Can u come up with something different?? Cmon lets try breaking the ODA CODE hehehe!!

Second one of these makes the most sense, predictable or not. As for Luffy not wearing one, he already has a Jolly Roger. Chance is it's not known to him (might not be well known in the East Blue) and even he was told about it now, I don't think he'd change his sign. Imo, it's just a different style of Jolly Roger which symbolizes the old era.

I don't have intention on breaking the "Oda code" because trying is useless and if I was actually right in my predictions then it wouldn't be half as fun reading the manga. :tem

Would that be Kidd's Jolly Roger?
I guess it's looks much more like Jack Skellington than anything: http://monitos.pcelectronica.cl/albums/userpics/10025/Jack%20Skellington.jpg

I think it's just a symbol of pirateering in the same way the skull and crossbones are in my opinion. I could go for the Roger theory, since a symbol of Roger is really a symbol of a pirate in the pirates age post-Roger, but not really anything beyond that.

I think its just simply a break away from the typical skull and cross bones that was overly used at the beginning of the series.

I think chances are that if they do mean anything at all is the 2nd more popular option. But is in no way significant to the storylines.

The Smiles represent the new age bought upon by Gol D Roger.
So option 2.

Wow good eye! That symbol is used by Pirates from North Blue, which is why Doflamingo has one like that. Maybe it's a Pirate made medicine, or just from North Blue. But I'm still surprised you noticed that....?!

The second one has my support, predictable or not
