Great! I appreciate your help. :)
If you want to edit one post or post as you find the mangaka is up to you.
Oda was the assistant for Shinobu Kaitani, Masaya Tokuhiro AND Nobuhiro Watsuki.

I remember two of them.
- Yoshihiro Togashi (Yu Yu Hakusho - Hunter X Hunter) → Yoshio Sawai (Bobobōbo Bōbobo)
- Tetsuo Hara (Hokuto no Ken) → Masanori Morita (Rokudenashi Blues - Rookies)
Yoshihiro Togashi -> Naoko Takeuchi
It is only for a while though. And it happened after Naoko completed Sailor Moon... I heard she quit because mangaka assistant's life is tough. :blink
Iwashiro Toshiaki (Psyren) -> Tamura Ryuhei (Beelzebub)
Yet another mangaka whose assistant become more popular/successful that he is. :amuse

Kinda suprised nobody linked into this website yet: http://www.ktr.to/Comic/assistant.html
(I always hate when I take on a project then stubble across that one website which can really speed this whole thing up)
It's not broken up by magazine (or era), so it's kinda hard to work through, but an English version of that would be very helpful.
From left to right, up to bottom
Spoiler show
@andyislands: Thank you so much! I'm sure there are A LOT more mangaka left to cover so this project is not quite done yet.
The next step is to organize these mangaka/assistants into categories, but that's for me to worry about so keep on posting!
Oh, it looks like I am late after everyone posted so many mangaka and their assistants. :(
Oh well, I guess I can help out with something else then, I can see what I can find but it is hard after everyone posted so much.
So if I understand this diagram correctly: http://www.ktr.to/Comic/assistant.png
Then Kishimoto wasn't assistant under any well known Mangaka (or at least not amongst one of this list which seems potentially Shounen exclusive to me? Dunno, there are only few English names) and hat at least five assistants who later published their own works?
Perhaps they won't give him so many assistants anymore and that's why he's continuisly complaining about having to do so much work :amuse
I updated the original post to include all the mangaka and their assistants in one thread. For simplicity I categorized them according to the head mangaka's demographic. It means that even though Mochizuki Akira's assistants (and their assistants) work primarily with shonen, I placed her chain under shoujo because that's her demographic.
We still don't have any examples for josei, and we need more shoujo artists!
Miura Kentaro's (Berserk) assistant, Mitsuhiku Kyuji, seemingly just started with Ookami no Kuchi: Wolfsmund (thx to Kaiten for telling us :D)
Also has a rather similar scenario, like Berserk it's a medieval european one.
Nobuyuki Anzai(Flame of Recca) -> Makoto Raikou(Gash Bell)
MASHIMA Hiro (Rave, Fairy Tail) -> YOSHIKAWA Miki (Yanki-kun to Megane-chan) + MIKUNI Shin (Spray King)
