Manji from Blade of the Immortal. The only way to kill him is decapitation so he does not mind gaping wounds or having limbs hacked off. It's part of his fighting style. And his nickname is the 100 man killer, after slaughtering the lord who retained him and his fellow samurai. He's actually a nice enough guy, if a bit gruff, who wants to redeem himself but he is one tough, mean looking guy.

The way Olivier looks and acts is pretty intimidating. Same with Buccaneer, but I'd rather deal with him than with Olivier. Alex also looks intimidating, but he really isn't. xD
Orochimaru is intimidating because of the way he looks. the way he acts is no less either, just by talking he can make you poop your pants silly. Madara's arrogant stare also makes him look intimidating, and the casual way he talks about wars and fighting doesn't help. Although Kakashi doesn't look intimidating, when you fight him, it's kinda hard not to be intimidated because the man's a freakin' genius. Jiraiya looks intimidating but he isn't unless angered or when needed to be serious. Tsunade doesn't look that intimidating, but when she's angry, then yeah, scarier than Orochimaru. X_X
Gutts from Berserk... It's been shown in the manga so far that he can scare opponents away by him just staring at him... Plus he is just a total badass...
He gets more intimidating as the manga progresses too. Since he started using the berserker armor he has looked increasingly grizzled too. Funny thing: as his appearence becomes more intimidating he has become a much more openhearted character. Fantasia arc Guts has come along way from the Guts of the Black Swordsman Arc.
Winters Socalo (D. Gray-man): I don't know how an ex-death row inmate got ushered to join the Black Order, but they're lucky to have someone like him on their side.
Johan (Monster)/Izaya (Durarara!!!)/Yokoya (Liar Game): They honestly don't look intimidating in appearance, but it's their mind and intellect that freaks me out. I'm more intimidated by someone whose appearance is deceiving.
Dou Haguro from Wolf Guy is extremely intimidating... even his image doesn't indicate just HOW fucked up the dude is, and he looks pretty scary.
Yuno from Mirai Nikki.
She has no qualms nor hesitation in killing even the most innocent in the most gruesome manner. To top it off, she's quick-witted, conscienceless and remorseless.
Remember when she tried to bury her classmates alive with cements just because they befriend Yukiteru?
Heaven helps me if i ever come closer than 10 meters to him or cast him the smallest glance. *shudder*
Madara, the way he stands there with his arms crossed and looks down on the kages as though they're small fries just radiates coldness. And the fact that he's willing to start off by dropping a meteor on you certainly shows he's the real deal. It's as though he's built for intimidation.
I'll add Charles zi Britannia, first and foremost its the way he carries himself, then comes the amount of power and authority that is there to back him up.

Yokoya from Liar Game.
He looks creepy enough, and the fact that he does anything to win makes him even more intimidating. This is only concerning the Liar Game so I don't even want to know how he deals with people in real life.

Johan from Monster. There isn't really any way to explain him, even though he is a fictional character - he gives me the chills.
