Well, it's certainly not true that adding v's to the beginning of personal pronouns is the only unique thing about Iva's speech in Japanese. For one thing, s/he lengthens the vowels in those pronouns...

I don't know, really... I don't really think too much about style of writing when I'm translating manga, I just try to write what seems natural, and of course what matches the feel of the original...

Yes, "hage" literally means "bald". Just like "dickhead" literally means someone has a dick instead of a head. They're just insults. I don't translate "hage" as "bald" in this context in English...
