Does anyone know the meaning of 素 here?

素(す-su) means the real (not artificial)
personality of someone.

The context is complex.
Girl A thinks she's partially the direct cause for girl B's injuries during an incident.
The guy who says this sentence asked girl A if she wanted to drop by the hospital to see how girl B is doing, since now she's out from a coma.
Girl B replies that until she has made things right and settled the score with those who caused the incident she won't go see girl B.
But! Girl A says that if he and the others have something to do at the hospital, they can obviously go and she'll wait outside of it.
So the guy says in his mind that やっぱりこちらが素なんだな talking about girl A.
Could it be a way to say that Girl A is a simple person? Pure? Not artificial like you say?
