There's lots of action in the mangaworld, because mankind is inherently peaceful and misses the action
But some just take it a bit far...
I was watching FMA the other day, and Mustang was on the verge of madness...the repeated burning of Envy's eyes was, well...cruel...
So what's your pick? Is there something that got to you, either because of the gore, or the extreme nature of the intent? Or maybe it was an emotional pain, or something important, but still painful(something like childbirth). Post any reasoning for the choice.

It is psychologically challenging to actually sit through Kago, I usually just flash quickly by the pages, some of his art is really cool, still the cruel stuff is way way too much.

Besides Kago, I think Umezu Kazuo's works are sometimes could twisting your stomach. I don't know if it is painful or not, but personally I have to be mentally prepared when reading horror mangas made by him.

From the top of my head: Battle Royale. I don't remember much of the first chapter (thankfully), but I was surprised that all these terrible things were happening right from the first chapter. I wasn't prepared to handle all of the gore and violence so soon.
Urasawa doesn't depict a lot of gore in his works, but there was one moment in Monster that kinda fits.
Any scene where a titan eats a person in Shingeki no Kyojin. I think it's due to the fact that they don't kill you and then eat you, they eat you... ALIVE.
The victims who get eaten feet first don't seem to die right away so imagine the pain and fear one feels right until their last breath. *shudder*
