In japanese culture, even a wife calls her husband by his title in front of others. When alone, it's not unusual to hear the wives calling them with san or sama.
It's considered disrespect if done differently.

A Kekkei Genkai takes time to master, and sometimes it costs you something (e.g. Itachi's sight).
Even if Naruto have a Kekkei Genkai, he would have to train hard to master it. And if he really has, surely it's not as simple to master -- the better...
