This is actually not true. In the first chapters, it is mentioned that there is an inner and an outer part of the tower or something along those lines. And that the people who decide to climb the (inner part of) the tower, come from the outer part. Only irregulars come from outside the tower...

Damn, that was actually a good chapter. I had forgotten how reading a good Bleach chapter felt, it's been so long! Even though it was out of character for Aizen to take on all the attacks, this wasn't a chapter where Bach trashed them with a finger or didn't anything and then trashed them.
