What you said is theoratically possible, but how I see it, it has absolutely no strategical value. If the Bodyguard and Sane Doctor get to find each other in the game and agree to protect each other, the Mafia has freewheel to kill anybody else, not fearing to be stopped there.

So then you have this Doc-Guard couple that publicly makes out with each other, everybody knows about it, but soon there are only two of them left and they are outnumbered by the Mafia.

But like mentioned, the Doctor can be Sane, Insane, Naive, and there is no real certainty about it. This chain can be easily broken by the Insanity or Naivity of the Doc, or other counters, such as Mafia Assassin for instance.

I agree with you, but there is one more role who can benefit from this. but the odds are very low to go all smooth. There is some ways from mafia benefits from this combo as well

I am guessing you are hinting at the Hider. Checking the description.

Hider (Townie): Each night the Hider may choose another person to hide behind. If somebody tries to kill the Hider while he is hiding then the kill fails. If the person that the Hider is hiding behind is killed then both of them die. If the Hider is hiding behind the person who tries to kill the Hider, then the kill succeeds. (GAME 3)

And so the Hider does not really benefit from if, as if he hides behind either the Bodyguard or Doctor, which the Mafia targets, then the bold part is still valid, with the result that the Hider dies alone (as he himself has not the protection from Doc or Guard).

but if the guard is protected by the doc and the doc is protected by the guard none of them will die so the Hider could piggyback.

That's what I am trying to explain is not the case. The Doctor - going to the basics to understand this role - treats patiences after the assault. So the attack on the Guard and Hider happened, he has medicine only for one - the guard which he chose in advance via PM, Hider dies, case solved. blablabla.

It's not the first and not the only Townie combo that has this effect. I don't see you for instance freaking out about Bulletproof Townie+ Hider, who can hide every night behind the Bulletproof? Kind of random, if you ask me.

I think it's because it's new and kind of untested. It's just like us dissecting role suggestions in the other thread. There's so many possibilities in this game, that's why it's good.

That's easy. Guard description "he becomes the target instead of the person he guards." A dies.
