This is why I mentioned a serial killer. If we in fact managed to wipe out the orange mafia and if demon didn't tangle with a pgo, we have to consider the possibility if a third killing party not associated with the mafia.

But even you have to admit the probability of one doctor is low. No one else is claiming it and if spirit was also protecting Xad its not farfetched to reason that James is insane and accidentally killed him without spirits protection. Or maybe the toss just went against Xad this time around since James claims to have only been protecting him every night.
GrySun's game(Ace Attorney) only had one Doctor. Having 2 Doctors isn't necessary.
