We have no idea, unlike other authors Miura doesn't give us too much clues about how much time has passed in the manga

This is the best estimation about his age that we have at our disposal.He could be older but at the moment I'm assuming he's at least 21 years old. Schierke looks like she might be 13 at best, and she's a bit underdeveloped compared to Isma, who could be around 15 actually .

There's another one (both are found on sk.net) where he looks like Laban. I think he would look more terrifying if he had a beard.
I wonder why Miura never drew him with one, considering Guts design is based on Kenshiro from Fist of the North Star.
