The Game:
It's pretty simple. Whenever one of the following events occurs in the manga, toss back the required number of drinks.
When a Noah cries, take two drinks.
Every time Komui throws a tantrum, take a drink.
Every time Kanda calls Allen "moyashi", take a drink.
When Allen is shown eating, take a drink.
When Kanda says "cheh", take a drink.
When Yoshi (the rabbit) appears, take a drink.
When Road cackles, take a drink.
When Lenalee cries, take a drink.
Whenever Miranda beats herself up for being useless, take a drink.
When Bookman hits Lavi, take a drink.
When Lenalee hits someone (who isn't an Akuma or Noah), take a drink.
When Allen and Lenalee are seen holding hands, take a drink.
When Cross' debts are mentioned, take a drink. (thanks koenosaki)
Every time Skin Boric mentions "sweets" take a drink. [thanks Crackpot27!]
When Cross hits on a woman, take a drink.
When Komui has a "sister complex" moment, take a drink. (thanks onutzikmik)
When Cross hits Allen take a drink and sulk. (thanks gorbal)
When Miranda blames herself for something take a drink and yell "I'M SORRY WORLD!" in your best Miranda voice (thanks luna_wolf)
when an exorcist gets injured in a battle then miraculously is ok, take a drink and celebrate (thanks black_crow)
When Allen gets a new upgrade, take two drinks.
When Komurin is seen or someone mentions Komurin, take two drinks.
When Lavi is shown eating, take two drinks.
When someone reminds Lavi of the responsibilities of a Bookman, take two drinks.
When Johnny appears, take two drinks.
When Baku breaks out in hives, take two drinks.

When Lavi goes "Strike" take two drinks and wish it was you/and yell "strike!" out loud at the next person you see. (thanks gorbal & luna_wolf)
When a named character (good/bad guy, doesn't matter as long as they had a name) dies, take two drinks and sing "Amazing Grace." bagpipes optional.
When the Earl cries, take three drinks.
When Krory emos over Eliade, take three drinks.
When Kanda is seen eating, take three drinks. (Of sake if possible.)
When Jerry appears, take three drinks and eat a five-course meal.
When Lulubelle appears, take three drinks.
When Lenalee is shown eating, take three drinks and eat a bag of Oreos and six pixy stix.
When Allen's eye acts up take a sip. (thanks black_crow)
Every time Allen cries, take one sip. (Because you'll be on the floor by vol 7 if you took a drink each time...
