Done Help with one line in Naoki Urasawa's Monster chapter 3

I'm tryin' to scanlate Monster's chapter 3 in greek but i find a little tricky the words on the second bubble (from right to left) of the very first panel of the very first page of this chapter.

I suppose there aren't many of you who can help me translate it directly into greek, so a translation from japanese to english would be pretty fine for me.

Finally, please don't bother tell me to look at the translation of an existing english Edited Manga of this chapter, cause i would like a second (third/fourth etc.) opinion from you guys!

Something like, taking the gunshot wound as something like a starting point or landmark to start the surgery in that region...though I don't know that much about surgery, so I'm not totally sure if it's correct though... ._."

Here is my transcription of the raw.
「創部は汚染されているから、十二分に消毒してくれ。」
「銃創を利用して皮切を行なう。」
「脳腫脹が起きてくる前に　迅速に挫滅脳の摘出をする。」

「銃創を利用して皮切を行なう。」 is the line in question correct?

I would translate as "We will use the bullet wound to start the incision."

I'm a little unsure how you should translate the 皮切. It generally means "the start/the beginning", but I would think it literally means "cutting the skin" in surgical contexts.

I believe 皮切　is the same as 皮膚切開 (skin incision).
