So obviously this big reveal leads to many questions regarding who's child Sarada actually is. The most obvious answer is she is the child of Sasuke and Karin and that Sakura is acting her mother for unknown reasons.
This is an extremely interesting revelation for a number of reasons. But we should start from the beginning of the manga.
Back in the day Orochimaru acquired numerous ninja to experiment on in large part because he was looking to access the powers of the Sage. Along the way he recruited Karin and Sasuke.
Now, given Orochimaru's rather inhuman methods, I don't think it's out of the realm of possibility to think he'd made Sasuke impregnate female ninja in order to create new "weapons" or future bodies for himself. Perhaps no ninja would be a better option than Karin because of her healing powers and Uzumaki lineage which gives her the advantage of extreme longevity. A child with Uchiha and Uzumaki blood then would the perfect body in many ways for Orochimaru.
This idea is back up by Suigetsu suggesting that Karin "did something" to Sasuke while they were both in the custody of Orochimaru. This prompted Karin to lash out and hit him. Karin would later think of drugging Suigetsu and Jugo so she could presumably seduce Sasuke and have sex with him.
So, was Karin pregnant with Sauske's child already in Naruto Shippuuden? The answer is obviously no.
In no way did Karin ever seem to sleep with Sasuke in their travels and she obviously never showed any signs of being pregnant. Not to mention Sarada's age makes no sense in this regard.
With that in mind, if Sarada is indeed Karin's daughter by Sasuke, we have a few ways she could have been birthed.
1) Orochimaru did somehow have Sasuke impregnate Karin and then took the fetus before it grew to term. It's possible that Sarada was locked in a test tube somewhere and her growth was stunted for some reason or another. Going to go with highly unlikely on this option.
2) Sasuke impregnated Karin during his travels away from Konoha then brought the child back to Konoha and married Sakura. These actions seem more likely, although this entirely out of Sasuke's character. Sasuke showed some signs of loving Sakura at the end of manga by poking her in the head. In terms of Sasuke's character that's actually HUGE in terms of affection for a number of reasons, mostly because of the symbolism it has with Itachi, the person he loved most. At the same time, he never showed any such feelings for Karin.
So to think Sasuke would then run off and bang Karin for shits and giggles makes little sense. What makes even less sense would be Karin abandoning Sasuke and her daughter since she loved him so much.
3) Karin used Orochimaru's lab equipement to impregnate herself. Highly unlikely. I don't forsee Karin going back to the hell holes that were Orochimaru's hideouts and putting Sasuke's kid in her. It is certainly possible, but she wanted Sasuke, not his child so this isn't really in line with her character.
4) Karin was pregnant with Sasuke's child by some sort of artificial pregnancy via some plot (more on this later) by a third party. WINNER.
I think that Sasuke had returned to Konoha to start a life with Sakura after his travels but one day either Karin showed up pregnant with Sarada and died, or Sarada showed up by herself in like a basket or something.
Now, the implications of this mean someone had access to Orochimaru's equipment (I'm sure he had DNA/Sperm/Egg samples from all his subjects) and then captured Karin and impregnated her. But who is that someone?
The obvious choices would be Kabuto and Orochimaru, however neither of them fit the bill here.
First off, Kabuto seemed to want to help people not hurt them, if he had any bad intentions left I don't think he'd have escaped Izanami so I'm going to rule him out.
Orochimaru is now less about "being the wind" and more about "watching the wind", so I don't see him doing this either.
So, unless it is an entirely new character, that would mean the person who did this would have to know all about the Sage and would also have to know about Orochimaru. Only one character would have that knowledge and moreso have that motivation to do something like capture Karin.
The answer: Zetsu.
There is a very strong possibility that Zetsu (white or black) still exists and would want to produce an Uchiha/Senju-Uzumaki hybrid for some purpose (likely creating some sort of Kaguya clone or getting access to a new Rinnegan).
Sasuke's "mission" thus could be hunting down Zetsu or getting to the bottom of where Karin is, who impregnated her, who killed her, etc.
Sasuke being a man who wants to revive his clan, obviously wouldn't throw Sarada away, and Sakura loving Sasuke and doing anything for him would obviously take Sarada as her own child and raise her.
Questions start to rise as to how it could take Sasuke so long to do what he is doing (he's been gone for like 12 years) and why he is the one doing it aside from having a person stake in everything?
